Make an Appointment

To ensure that your visit to our practice is timely and that you receive efficient service from our staff, and that our physicians can provide you with the most appropriate and expeditious care, we ask that you provide us with the following for your visit:
  • Insurance card(s)
  • Referral forms from your primary care provider if required by your insurance plan
  • Pertinent x-rays, lab results and previous records that you may have relating to your condition for which you are seeing our physicians
  • Your personal medical history, including a list of your medications and allergies

Office Delays
Sometimes for reasons beyond their control, our medical office will run behind schedule. Physicians, office emergencies, and surgical emergencies impact schedules and result in unpredictable waiting periods. We make every effort to maintain our schedule and minimize any inconvenience to you. However, emergencies do occur and people do not schedule their illnesses. If a delay occurs, we will inform you as soon as possible and we will gladly reschedule your appointment if you prefer not to wait.
